Important Notes:

• Filling out of this form shows the tournament that you are available.  It does not commit you or the tournament to an assignment at this time.
• Assignments will be made based on the needs of the tournament, available games, referee experience, referee availability, and fit of the game to the referee.
• Once games are assigned you are committed to them and you are expected to officiate the matches assigned.  Due to other league priorities the tournament assignor shall reserve the right to release a referee from some or all of his/her assignments to cover higher priority play-off matches within the league.  Referees MAY NOT accept any other assignments unless and until the tournament assignor has released him or her from assigned Commissioner's Cup matches.
• All tournament referees are EXPECTED to be in PROPER UNIFORM.  No sweat pants, sweat shirts (visible), or shoes other than black may be worn.  These items may be allowed, at the discretion of the tournament assignor, if the weather is unusually cool and/or wet.  Please see the Instructions for Referees page for the proper uniform standards.
